  • Request Confidential Communications: You may request that we communicate with you in a particular way (for example, via mail instead of phone) to ensure your privacy.

  • Receive an Accounting of Disclosures: You can ask for a list of disclosures we have made of your PHI, excluding disclosures for treatment, payment, healthcare operations, or those made with your authorization.

  • Obtain a Paper Copy of This Notice: You have the right to request a paper copy of this Notice at any time, even if you initially agreed to receive it electronically.

To exercise any of these rights, please submit a written request to our office. We will respond promptly in accordance with HIPAA regulations.

Complaints and Inquiries

If you believe your privacy rights have been violated, you have the right to file a complaint without fear of retaliation. Complaints can be filed with:

  • Our Privacy Officer at Dental Made Easy.

  • The U.S. Department of Health and Human Services, Office for Civil Rights.
